Dr Melanie Tan is a geriatrician at Gleneagles Hospital, Singapore.
Her clinical interests include age-related conditions, from dementia and memory loss to frailty, falls, chronic disease management and complex medication use.
She currently serves as a visiting consultant on the National University Health System’s At Home programme. She is also a visiting consultant at Ng Teng Fong General Hospital.
Dr Tan graduated from Monash University, Australia. She obtained membership of the Royal College of Physicians, UK, followed by fellowships from both the Academy of Medicine, Singapore and the Royal College of Physicians, UK. She has also completed a graduate diploma in palliative care.
Throughout her career, she has been an advocate of innovative models of geriatric care. She led the development of the HOME Unit, a ward-based programme designed to keep hospitalised seniors active, alert and engaged. By preserving mobility and preventing complications such as deconditioning and confusion, the initiative became a catalyst for broader hospital improvements.
She also guided the implementation of the Elder Bundle, a set of coordinated best practices that has since become a benchmark in team-based geriatric care across departments. She is also certified in homecare and palliative care.
She is the vice-president of the Society for Geriatric Medicine Singapore.
Dr Tan is a recipient of the Silver Ambassador Award.
